All Care 4 Kids Applicants must turn in forms to Jenn Vega no later than May 26, 2017. Forms must be submitted through Wakeman Boys & Girls Club Smilow-Burroughs Clubhouse. Parents cannot send them in on their own.
All applicants are required to complete 2016-2017 membership payment per child (if membership has expired). Care 4 Kids assistance does not apply towards Wakeman membership.
Applicants will be notified via email and phone of their application status (please make sure Wakeman has correct email and phone number on file). Parent(s)/guardian(s) are responsible for any portion of fees not covered/approved by Care 4 Kids. Once determination letters are received by Care 4 Kids, balances will be invoiced to parents who will then have 14 days to make a payment. Payments not received may result in suspension of camp participation.
